**Contexte:**

L'endoscopie en lumière blanche (ELB) conventionnelle est utilisée de routine en endoscopie digestive. Cette technique a cependant une capacité limitée de caractérisation complète des motifs vasculaires d'une lésion ou de détection d'une inflammation subtile. Cela peut entraîner un délai de traitement ou un traitement sous-optimal d'une condition. La chromoendoscopie traditionnelle est utilisée pour renforcir le contraste pour meiux définir les contours et caractériser des lésions pour en prédire l'histologie; elle présente cependant ses propres limites, incluant l'usage de colorants coûteux, l'application inégale des colorants et la longue durée de l'intervention.

Pour préserver la qualité supérieure de l'image obtenue par chromoendoscopie sans devoir utiliser des colorants, une technique de chromoendoscopie virtuelle a été développée. Cette méthode offre en temps réel, au cours de l'intervention endoscopique, des contrastes numériques qui permettent de visualiser les surfaces des muqueuses et les motifs vasculaires des lésions pathologiques. Des lésions subtiles peuvent tout de même passer inaperçues avec  
cette technique.

**Aperçu de i-SCAN et OE:**

Il y a plus de huit ans, PENTAX Medical a introduit la technologie i-SCAN, une technologie offrant une amélioration de l'image en temps réel au cours des interventions endoscopiques. La technologie OE a depuis été mise au point pour améliorer davantage l'imagerie de fort contraste générée par i-SCAN. i-SCAN n'utilise que de la lumière blanche pour illuminer les structures alors qu'OE fait appel à des filtres optiques pour créer un spectre de longueurs d'onde continu en reliant les pics du spectre d'absorption de l'hémoglobine.

OE est une combinaison unique de chromoendoscopie numérique et optique mettant en évidence les détails de la surface et de la vascularisation des muqueuses pour un diagnostic in vivo. Pour un seul médecin, l'usage concomitant de i-SCAN et de OE permet d'obtenir « plusieurs plateformes multimodales de chromoendoscopie électronique au sein du même équipement. »

**Usages actuels:**

*Tube digestif supérieur:*

De nombreuses études ont examiné l'usage de i-SCAN et de OE dans le tube digestif supérieur. Cammarata et ses collaborateurs ont trouvé que i-SCAN offrait une détection à 100 % exacte des motifs d'atrophie marquée des villosités et à 90 % exacte de l'atrophie partielle et des motifs normaux des villosités de la muqueuse duodénale. Poon et ses collaborateurs ont trouvé une corrélation significative (P < 0,00001) entre l'évaluation histologique de Marsh et l'évaluation endoscopique à l'aide de i-SCAN haute-définition avec immersion dans l'eau (IE), suggérant que l'endoscopie i-SCAN-IE reflète la gravité histologique de la maladie céliaque avec plus d'exactitude que ce qui est possible avec l'ELB conventionnelle.

Robles-Medranda et ses collaborateurs ont étudié la capacité des technologies i-SCAN, OE et de l'endoscope MagniView de Pentax Medical à diagnostiquer la maladie du reflux non érosif (NERD en anglais) par la détection de lésions pathognomoniques minimales de l'oesophage (LMO) et de boucles capillaires intra papillaires (BCIP). Des BCIP ont été observées chez 94,4 % des patients avec NERD et 38 % des patients du groupe contrôle (P < 0,05). OE avec MagniView a permis de détecter les LMO et prédire les NERD avec une plus grande sensibilité et précision comparativement au "gold standard" de la mesure d'impédance du pH.

*Tube digestif inférieur:*

La technologie d'amélioration de l'image numérique a également démontré des résultats impressionnants au niveau du tube digestif inférieur. Dans une revue effectuée par Hancock et ses collaborateurs, i-SCAN a été utilisé pour diagnostiquer et traiter des adénomes tubulaires plats, des cancers, des cellules squameuses anales et des adénocarcinomes du rectum. La technologie est également utilisée pour caractériser des polypes, évaluer des colites microscopiques et caractériser des lésions du colon chez les patients avec MCI.

Dans une étude comparant l'utilité de i-SCAN par rapport à l'ELB chez les patients avec colites ulcéreuses, Iacucci et ses collaborateurs ont trouvé une corrélation significative (P < 0,0001) entre les sous-scores endoscopiques de Mayo et les motifs généraux en haute-définition de la muqueuse et de la vascularisation. Les auteurs ont déduit que i-SCAN peut détecter des lésions et de l'inflammation subtile qui ne sont pas autrement détectés par ELB conventionnelle, suggérant ainsi que i-SCAN est préférable pour évaluer la guérison de la muqueuse de ces patients. Il a également été montré que i-SCAN avait une sensibilité de 100 % et une spécificité de 83,3 % dans la détection des lésions dysplastiques des patients avec MCI chronique.

**Usages futurs:**

Les technologies i-SCAN et OE peuvent être utilisées pour faciliter le diagnostic in-vivo de conditions comme l'oesophage de Barrett, la maladie céliaque et les polypes colorectaux. L'usage de tests supplémentaires, comme la manométrie et la vérification du pH, peut s'avérer inutile. Les patients avec MCI recevront un traitement ciblé, évitant ainsi les résections inutiles du colon et réduisant les complications et les hospitalisations.

La politique de « réséquer et éliminer » introduite par la American Society for Gastrointestinal Endoscopy (ASGE - société américaine pour l'endoscopie gastrointestinale) recommande que les polypes colorectaux à faible risque devraient être éliminés après résection plutôt que d'être soumis à une évaluation pathologique. Par l'entremise d'une visualisation des micro vaisseaux et d'une caractérisation des lésions améliorées, i-SCAN avec OE facilitera la mise en oeuvre de cette politique par les cliniciens.

i-SCAN et OE peuvent également être utilisés pour guider la résection endoscopique de lésions chez les patients avec MCI en fournissant une caractérisation de haute définition des marges et de l'histologie des lésions, servant ainsi potentiellement de traitement permettant d'épargner des organes. i-SCAN et EO peuvent également servir à évaluer l'activité de la maladie(guérison de la muqueuse) chez les patients avec MCI; une telle évaluation permettrait de guider la gestion thérapeutique.

**Conclusion:**

Les systèmes i-SCAN et OE de PENTAX Medical ont montré des aptitudes supérieures à l'ELB conventionnelle dans le diagnostic des lésions du tube digestif supérieur et inférieur, surtout chez les patients avec MII. D'autres études sur la configuration et la conception de ces technologies sont nécessaires pour établir leur utilisation normalisée pour diagnostiquer les adénomes et les lésions malignes du tube digestif.
